Tuition Assistance Process
Tuition Assistance Application Checklist for 2013-2014
Please make sure you complete all the steps below to ensure your materials will be reviewed by our tuition assistance committee.
Note: Deadline for applying for tuition assistance through the
FACTS Grant & Aid Assessment program is March 15, 2013.
□ Past due balance of $ must be paid in full; or payment arrangements must be in effect. (Send to Tuition Office)
□ Include: Registration, Activity Fee, and Tuition Assistance Application Fee. [$500, $250 or $150, as appropriate] (Send to Tuition Office)
□ Payment arrangements must be in place for 2013-2014 (“HANC Tuition Payment Preference Form” must be completed and signed). If an employer or 3rd party is paying HANC directly, they must confirm this agreement in writing. (Send to Tuition Office)
□ Enrollment contract must be signed by both parents. If tuition assistance is awarded, a revised contract will automatically override the original signed contract. (Send to Tuition Office)
□ “TUITION ASSISTANCE BOND AGREEMENT” must be signed. (Send to Tuition Office)
□ “TUITION ASSISTANCE ACKNOWLEDGEMENT OF OBLIGATIONS” must be signed. (Send to Tuition Office)
□ “FACTS Grant and Aid Application” must be completed online by March 15, 2013 at https://online.factsmgt.com/signin/3FLN5.
□ “Supplemental Application for Tuition Assistance” must be completed; it can be accessed via hanc.org. (Send to Tuition Office)
□ Copies of your complete personal and business (if applicable) 2012 Federal Tax Returns are to be mailed or faxed to FACTS at 1-866-315-9264. If your 2012 Tax Return(s) are not available, submit your 2011 Tax Return AND 2012 W-2s. Note that your application may not be evaluated by HANC’s Tuition Assistance Committee without your 2011 or 2012 tax return. (Send to FACTS)
□ Print an entire credit report (from only one of the 3 credit bureaus) for both parents by logging onto www.annualcreditreport.com (everyone is entitled to one free credit report per year). (Send to Tuition Office)
□ Proof if you have applied for or are receiving public assistance (including unemployment benefits, etc.). (Send to Tuition Office)
IT IS YOUR RESPONSIBILITY TO MAKE SURE ALL THE NECESSARY PAPERWORK IS SUBMITTED TO FACTS AND THE TUITION OFFICE.
YOUR APPLICATION WILL BE EVALUATED ONLY WHEN ALL PAPERWORK IS COMPLETE.
